If you do not see an application here for the coverage you are interested in, call our office and we will take care of you either over the phone or by mailing or faxing you an application.

Mortality Application: Includes application page, Statement of Health and Veterinary Examination Form. If you are applying for Loss of Use coverage, you must also download the Loss of Use Veterinary Examination Form. We have included the Mortality Application as a Word document for those customers unable to open a .pdf file.

Commercial Equine Liability Application: This application is for most equestrian operations desiring liability only without any property coverage. Care Custody and Control may be included if applicable. If you have Day Camps, Pony Rides or Carriage Rides, you must also download and complete the Liability Supplement. This application cannot be used for rentals, outfitters or guides. Please contact our office and we will provide the appropriate application via mail or fax.
